Understanding equipment benefits and your costs

Make sense of network status, deductibles and the questions to ask before confirming an order.

Start with your particular plan

Two people requesting the same item can have different benefits and different costs. The plan type, supplier arrangements, available benefits and required documentation can all affect the outcome. Use the name of your actual plan when speaking with its member-services team. A familiar insurance-company name by itself may not identify which rules apply to you.

For example, Original Medicare and a Medicare Advantage plan do not have identical processes. Employer plans and Medicaid programs can have their own requirements. An online statement about a product category should be treated as a starting point for questions, not a personal estimate.

Keep network status separate from product coverage

A plan may cover a type of equipment while applying different rules to a particular supplier. Medrax operates as an out-of-network supplier, so ask how that affects the request. Confirm whether your plan has out-of-network benefits, whether an authorization is needed and whether any benefit limits apply to the product.

For Medicare questions, enrollment, participating-supplier status and acceptance of assignment are distinct concepts. Do not assume that a supplier’s ability to discuss Medicare means the supplier accepts assignment for your order. Ask for a clear answer about the arrangement that applies before agreeing to receive the item.

Ask for the costs in plain language

Ask what amount may be your responsibility and what is included in that explanation. A deductible, copayment, coinsurance, noncovered accessory or difference related to supplier arrangements can change what you owe. A benefit estimate may depend on information that is not final yet, so ask which parts are confirmed and which remain subject to review.

It can help to write down the product, quantity, date of the conversation and any reference number the plan provides. If the order changes, such as a different model or quantity, ask whether the cost explanation also needs to be updated.

Understand the next decision

A prescription, a benefits review and a prior authorization serve different purposes. None should be presented as a blanket guarantee that every charge will be paid. Ask what is needed next, whether there are alternative arrangements and when you will have enough information to decide whether to proceed.

If you later receive a statement you do not understand, compare the dates, product and quantities with the order information. Contact the supplier and plan for an explanation rather than assuming the first document you receive is a final bill. Keep your questions specific so the team can identify the charge you mean.

  • Does this request have out-of-network benefits?
  • Which product and quantity does the estimate describe?
  • Is this equipment a purchase or a rental?
  • What information could change the expected cost?
