Review the item and your actual plan
Availability and coverage are reviewed for each request. An insurance card, prescription or product listing does not by itself establish that an item is covered. The team can discuss the information needed to review your particular request.
Medrax is an out-of-network supplier
Network status affects how a plan may handle an order. Ask whether your plan includes applicable out-of-network benefits, whether authorization is needed and how your costs will be explained. We do not claim that every plan or every product is eligible.
Ask about your financial responsibility
Before confirming an order, ask about any expected deductible, copayment, coinsurance or noncovered amount and what remains uncertain. A benefits estimate is not a guarantee of final payment. Original Medicare, Medicare Advantage, Medicaid and commercial plans have different requirements.
Product-specific requirements matter
A CGM, brace and disposable supply may follow different rules. Your clinician provides the required prescription and clinical documentation. Medicare enrollment, acceptance of assignment and plan network participation are different facts; ask how the relevant arrangement applies to your order.
Can this website approve my insurance?
No. The preparation guide does not contact an insurer or submit a request. Call Medrax to discuss a benefits review.
Will I know the exact product before an order?
Ask the team to confirm the manufacturer, model, size or side where relevant, package quantity and any expected cost before you proceed.
Should I email my insurance card?
Please call first to ask for the appropriate way to provide sensitive information. Do not send insurance cards or medical records to the general email address.
