Preparing for a CGM benefits review

Know what to ask about your plan, prescription and current equipment before you call.

Write down the goal of your request

You may be asking about your first continuous glucose monitor, changing systems or finding replacement supplies for equipment you already use. Start by stating which of these applies. If your clinician has named a system, keep the exact name available. If you are still considering options, say so; a benefits conversation does not replace clinical product selection.

It is also useful to note whether you have an order already and which office manages your diabetes equipment. You do not need to collect or email all of your medical records just to ask how the process works.

Understand why there is a review

Insurance benefits depend on the plan and the individual request. Medicare’s public guidance says a CGM may be covered for a person with diabetes when specified conditions are met, including insulin treatment or a history of problematic low blood sugar, a practitioner order and evaluation, and sufficient training for the patient or caregiver. This is a general outline, not a determination that you qualify.

Other plans may use different requirements and channels. Ask which benefit applies to the product and whether the proposed supplier arrangement is covered. Medrax’s out-of-network status should be part of that discussion before an order is confirmed.

Let the team explain the document route

Ask which prescription and supporting records are needed for the exact product. Your treating clinician provides medical documentation; a patient website questionnaire cannot establish medical necessity. If a visit or updated record is required, clarify who needs to arrange it and which office should receive the request.

Before sending an insurance card, prescription or chart note, ask how it should be transmitted. The preparation guide on this website does not receive documents or submit a benefits request. It is a way to organize general questions for a conversation with the team.

Leave the conversation with a clear next step

Ask what remains outstanding, who will work on it and how you can follow up. Confirm whether the team is discussing a receiver, sensors, related supplies or a combination. If you already have equipment, ask whether that affects the request and identify the exact device rather than describing it by color or appearance.

Before proceeding, request an explanation of any expected cost and available delivery arrangements. If you are close to running out of supplies, tell your clinical team and supplier promptly so they can discuss your situation. Do not assume that an initial enquiry guarantees an approval or a delivery date.

  • Which exact item is being reviewed?
  • What information is still needed from my clinician?
  • How does the supplier’s network status affect me?
  • How can I follow up on the request?
