Getting medical equipment through insurance

A practical starting point for product questions, benefits review, paperwork and delivery.

Begin with the item you need

You do not need to understand billing codes to start a conversation about medical equipment. Begin with the product your clinician recommended, a model name from a prescription, or the item you already use. If you are unsure of the name, explain what information you have and ask how to identify it. A photo of packaging can be useful later through an appropriate communication channel.

For a replacement, look for the manufacturer, model, size and part number on the package. These details help distinguish a complete device from an accessory. They also reduce the chance of discussing a product that looks similar but works differently.

Ask what the benefits review will cover

A benefits review looks at the requested product and your particular plan. Ask whether the item falls under a medical benefit, whether authorization is needed and how supplier network status affects your costs. Medrax operates as an out-of-network supplier. That detail should be part of the conversation from the beginning, even if another supplier has previously provided a similar item.

A covered product category is not a promise that every model or every request will be paid. Your prescription, documentation, benefit limits and individual circumstances may all affect the answer. Keep notes on any questions you want clarified before deciding to proceed.

Bring your care team into the process

Your clinician determines the medical need and provides the required order. Some requests also need supporting records or a recent visit. Ask which office should receive the paperwork request and whether you need to arrange an appointment. Do not change your treatment or select a different device simply to complete an online form.

If several offices are involved, identify the clinician managing this equipment. A clear contact makes it easier to understand where an outstanding prescription or record may be waiting.

Confirm the order before delivery

Before an order is confirmed, ask for the exact item, package contents, expected financial responsibility and delivery arrangements. Clarify whether equipment is purchased or rented, whether setup or fitting is needed and who will answer questions after arrival. If a cost or delivery detail is still uncertain, ask what needs to happen to resolve it.

The online guide on this website helps you prepare for a call; it does not send an insurance request. Call Medrax at (480) 550-8566 to discuss the next step. Have your questions ready, and ask the team how to provide any sensitive documents safely.

  • What is the exact product and quantity?
  • What documents or authorization are still needed?
  • What might I owe, and when will that be explained?
  • Who should I contact about delivery, use or a replacement?
